- Days 91-150: Medicare pays all eligible expenses after patient pays a per day co-payment which is $800 a day.
- Days 151 or more: Medicare pays nothing and you pay all costs.
In regards to Skilled nursing confinement when you are hospitalized for at least three days and enter a Medicare-approved skilled nursing facility within 30 days after hospital discharge and are receiving skilled nursing care Medicare pays all eligible expenses for the first 20 days; then all eligible expenses for days 21-100 after patient pays a per day co-payment. After 20 days, $200.00 a day co-payment.

For incurred medical expenses from physicians services for in-patient, outpatient, medical/surgical services; physical/speech therapy and diagnostic tests, Medicare covers 80% of approved amount after you pay the annual Part B deductible of $226.00.
You are then responsible for the remaining 20% of the approved amount. Clinical laboratory services like blood tests or urinalysis Medicare generally covers 100% of the approved amount.

Benefits of Medicare Supplement Plans
Medicare Supplement Insurance plans have different benefits that are covered. Premiums vary among Insurance companies , but the benefits of each standard Medigap plan are always the same. You pay a monthly premium to insurance company in addition to your Medicare Part B premium. The cost of Medigap policy depends on the type of plan you buy, the insurance company, your location, your age and tobacco use.
For Ex. Plan G for a female age 70 zip code 18914 non-smoker with Aetna would be $143.86 per month. Plan G for the same client with Mutual of Omaha would be $200.86.
As you can see there is quite a difference in premium. Each company has different underwriting questions which is also another factor in deciding on a Medicare Supplement company based on your health and medications you may take.
Please see attached grids for each plan’s benefits. C&F phasing out for new Medicare eligible effective 1/1/2020.
